Hair Loss (Alopecia) & Dandruff

Hair loss (alopecia) is one of the most psychologically distressing conditions affecting both men and women. Approximately 50% of men experience significant hair loss by the age of 50, while 25% of women are affected at some point in their lives. On average, a healthy adult sheds 50–100 hairs per day as part of the normal growth cycle; hair loss becomes clinically significant when shedding exceeds this or when regrowth fails to keep pace.

Dandruff (seborrheic dermatitis of the scalp) is even more prevalent, affecting up to 50% of the global population at some point. It manifests as flaking of scalp skin, often with itching, and ranges from mild dry flaking to greasy, yellowish crusts associated with an inflamed, oily scalp.

Types of Hair Loss

Androgenetic Alopecia

The most common form (male and female pattern baldness). Driven by the hormone DHT (dihydrotestosterone) causing progressive miniaturisation of hair follicles. In men: receding hairline and crown thinning. In women: diffuse central thinning.

Alopecia Areata

Autoimmune condition causing patchy, well-defined areas of hair loss on the scalp or anywhere on the body. The immune system mistakenly attacks hair follicles. Can progress to total scalp (alopecia totalis) or total body (alopecia universalis) hair loss.

Telogen Effluvium

Diffuse, generalised hair shedding triggered by physiological stress: high fever, major surgery, childbirth, severe emotional shock, crash dieting or significant illness. Follicles prematurely enter the resting (telogen) phase; shedding occurs 2–4 months after the trigger.

Scarring Alopecia

Permanent hair loss caused by destruction and replacement of follicles with scar tissue; due to infections, autoimmune disorders (lichen planopilaris, discoid lupus) or physical trauma. Hair regrowth is not possible in affected areas.

Causes of Hair Loss

Systemic Causes

  • Hormonal imbalance: Excess DHT (androgenetic alopecia), thyroid dysfunction (both hypo- and hyperthyroidism), PCOD, post-menopausal oestrogen decline
  • Nutritional deficiency: Iron, ferritin, zinc, biotin and B vitamins (especially B12) are essential for hair follicle metabolism
  • Autoimmune disease: Alopecia areata, systemic lupus erythematosus
  • Post-partum hair loss: Telogen effluvium triggered by the dramatic hormonal drop after childbirth

Local & External Causes

  • Scalp infections: Tinea capitis (ringworm of the scalp) caused by dermatophyte fungi; causes patchy, scaly hair loss in children
  • Psychological stress: Severe acute or chronic stress triggers telogen effluvium
  • Medications: Chemotherapy, anticoagulants, retinoids, beta-blockers, antidepressants can cause hair loss as a side effect
  • Genetic predisposition: Family history on either parent’s side strongly predicts androgenetic alopecia

Dandruff — Seborrheic Dermatitis

Dandruff is caused by the overgrowth of a naturally occurring yeast called Malassezia globosa on the scalp. This yeast feeds on scalp sebum and produces oleic acid, which penetrates the scalp barrier and triggers an inflammatory response in susceptible individuals, accelerating skin cell turnover and producing the characteristic flakes.

Triggers & Aggravating Factors

  • Oily scalp (seborrhoea) providing abundant food for Malassezia
  • Psychological stress — a well-documented trigger for seborrheic dermatitis flares
  • Cold, dry weather reducing protective barrier function
  • Hormonal changes increasing sebum production
  • Infrequent shampooing allowing sebum and yeast to accumulate
  • Neurological conditions (Parkinson’s disease, HIV) predispose to severe seborrheic dermatitis

Features Distinguishing Types of Dandruff

Dry dandruff: Small, white, powdery flakes; scalp feels tight and itchy; worse in winter and with harsh shampoos.
Oily dandruff: Yellow, greasy, larger flakes stuck to the scalp; associated with seborrheic dermatitis; can affect eyebrows, nose creases and ears.

Homeopathic Approach to Hair Loss & Dandruff

Homeopathic treatment of hair loss and dandruff is holistic, addressing both the scalp and the systemic factors driving the condition. Unlike minoxidil (which requires indefinite use and causes shedding on discontinuation) or anti-dandruff shampoos (which suppress the Malassezia temporarily), constitutional homeopathic remedies target the underlying predisposition.

The homeopathic approach achieves:

  • Improved scalp microcirculation — better blood supply to follicles means stronger, better-nourished hair shafts
  • Hormonal balance — addressing DHT sensitivity, thyroid dysfunction or PCOD at the constitutional level
  • Immune modulation — in autoimmune alopecia areata, calming the aberrant immune attack on follicles
  • Scalp ecology normalisation — reducing sebum excess and correcting the scalp environment that allows Malassezia to thrive
  • Systemic nutritional support — remedies such as Silicea and Calcarea improve absorption of minerals essential for hair growth

Treatment duration varies: telogen effluvium typically responds within 3–6 months, while androgenetic and autoimmune alopecia require longer constitutional treatment of 12–24 months with progressive improvement.
