What is Acne Vulgaris?

Acne vulgaris is a chronic inflammatory disorder of the sebaceous (oil) glands, characterised by seborrhoea (excess sebum secretion), comedones, papules, pustules and — in severe cases — cystic nodules that leave permanent scarring. It is primarily a disease of adolescence, driven by hormonal changes that enlarge the sebaceous glands and increase sebum output, but it can persist well into adulthood.

The pilosebaceous unit (hair follicle + sebaceous gland) becomes blocked when excess keratin and sebum combine to form a plug. This creates the ideal anaerobic environment for Cutibacterium acnes (formerly Propionibacterium acnes) to proliferate, triggering the immune-mediated inflammation responsible for red, painful pimples and cysts.

Rising testosterone at puberty stimulates sebaceous gland hypertrophy and excess sebum secretion, explaining why acne is so prevalent in teenagers of both sexes.
