What is Duodenal Ulcer?

A duodenal ulcer is an open sore (ulcer) that develops in the lining of the duodenum — the first part of the small intestine immediately beyond the stomach. When ulcers occur in the stomach lining, they are called gastric ulcers. Together they are known as peptic ulcer disease (PUD), affecting approximately 10% of the world’s population at some point in their lives.

The stomach and duodenum are protected from gastric acid and digestive enzymes by a layer of mucus. When this protective barrier is compromised — whether by bacterial infection, medication or lifestyle factors — acid erodes the underlying tissue, forming an ulcer. Duodenal ulcers are more common than gastric ulcers (approximately 4:1 ratio) and tend to affect younger individuals.

Causes of Duodenal Ulcer

Primary Causes

  • Helicobacter pylori (H. pylori) infection: The most common cause, responsible for approximately 80% of duodenal ulcers. This bacterium burrows into the mucosal lining, weakening its protective defences and allowing acid injury.
  • NSAIDs and aspirin overuse: Non-steroidal anti-inflammatory drugs inhibit prostaglandin synthesis, stripping the protective mucous lining of the stomach and duodenum. Regular users face a 3–4-fold increased risk of peptic ulcers.
  • Acid hypersecretion (Zollinger-Ellison syndrome): Rare tumours (gastrinomas) secrete excessive gastrin, driving massive acid production.

Contributing Factors

  • Stress: Severe physiological stress (major surgery, burns, critical illness) can cause stress ulcers; psychological stress exacerbates symptoms
  • Smoking: Impairs mucosal healing, increases acid secretion and doubles the risk of duodenal ulcer
  • Alcohol: Directly irritates gastric mucosa and increases acid secretion
  • Blood group O: Individuals with blood group O have a genetic predisposition to duodenal ulceration
  • Dietary habits: Spicy, acidic or fatty foods worsen symptoms in those with existing ulcers

Symptoms of Duodenal Ulcer

The hallmark symptom is a burning or gnawing pain in the upper abdomen (epigastrium), typically with the following pattern:

Classic Presentation

  • Pain begins 2–3 hours after eating (when the buffer of food is gone and acid contacts the ulcer directly)
  • Pain is relieved by eating or taking antacids (unlike gastric ulcers, where food may worsen pain)
  • Characteristically wakes the patient at night (2–3 am) due to acid secretion without food buffer
  • Periodic episodes lasting weeks followed by symptom-free intervals

Associated Symptoms

  • Nausea and occasional vomiting
  • Bloating and feeling of fullness
  • Belching and heartburn
  • Loss of appetite and unintentional weight loss
  • Dark or tarry stools (melaena): Indicates bleeding from the ulcer — requires urgent medical attention
  • Haematemesis (vomiting blood) in severe cases

Types of Peptic Ulcer

By Location

  • Duodenal ulcer: In the duodenum (most common)
  • Gastric ulcer: In the stomach lining
  • Oesophageal ulcer: In the lower oesophagus, often related to GERD

By Duration

  • Acute ulcer: Sudden onset, often stress or NSAID-induced; may bleed or perforate rapidly
  • Chronic ulcer: Long-standing; associated with H. pylori; may penetrate surrounding organs

By Complication

  • Bleeding ulcer: Erosion of a blood vessel; most common complication
  • Perforated ulcer: Full-thickness hole into the peritoneal cavity; surgical emergency
  • Obstructing ulcer: Scarring causes gastric outlet obstruction

Homeopathic Approach to Gastric Disorders

Homeopathic treatment of peptic ulcer disease differs fundamentally from conventional acid suppression therapy. Rather than blocking proton pumps or neutralising acid indefinitely, homeopathy addresses the underlying constitutional weakness that makes the individual susceptible to mucosal injury in the first place.

The homeopathic approach works at multiple levels simultaneously:

  • Reducing acid hypersecretion by correcting the constitutional tendency toward excess gastric acid production
  • Healing the mucosal lining by stimulating the body’s own tissue repair mechanisms
  • Addressing susceptibility to H. pylori by strengthening overall immune and mucosal defences
  • Eliminating lifestyle triggers by treating the anxiety, irritability or dietary compulsions that drive ulcer formation
  • Preventing relapse through deep constitutional treatment rather than symptomatic suppression

Patients often report that homeopathic treatment allows them to gradually reduce and eventually discontinue long-term proton pump inhibitor (PPI) therapy under medical guidance.

Homeopathic Remedies for Gastric Disorders

Nux Vomica

Ulcers from overindulgence in spicy food, alcohol and stimulants. Stomach pain worse in the early morning. Nausea without vomiting; sour or bitter taste. Irritable, ambitious, type-A constitution with sedentary habits and overwork.

Lycopodium

Bloating and flatulence immediately after small quantities of food. Acidity worse 4–8 pm. Craving for sweets and warm food. Anticipatory anxiety. Liver-based constitution; wakes feeling hungry but quickly feels full.

Argentum Nitricum

Craving for sweets which worsen gastric symptoms. Excessive belching, flatulence and bloating. Anticipatory anxiety causing diarrhoea. Burning, constricting epigastric pain. Hurried, impulsive personality prone to sudden diarrhoea before important events.

Phosphorus

Burning pain in the stomach and oesophagus. Craving for cold drinks which temporarily relieve but are then vomited when they become warm in the stomach. Haemorrhagic tendency — bleeding ulcers. Sensitive, sympathetic constitution.

Robinia

Intense acidity with frontal headache. Sour vomiting at night, causing waking from sleep. Hyperacidity with regurgitation of intensely sour fluid. Acid erodes tooth enamel. The leading remedy for nocturnal acid reflux and sour stomach.

Natrum Phosphoricum

Excess acid with sour belching and sour vomiting. Yellow coating on the tongue (key symptom). Gastric flatulence and waterbrash. Suited to individuals with lactic acid excess; often useful after excessive sugar or dairy consumption.

Arsenicum Album

Burning pain in the stomach relieved by warmth (warm drinks, hot water bottle). Intense anxiety and restlessness with gastric complaints. Nausea, vomiting and diarrhoea occurring together. Fastidious, chilly, anxious constitution; worse after midnight.

Carbo Vegetabilis

Excessive flatulence and gas causing upward and downward distension. Slow, impaired digestion with heaviness after eating. Acidity and heartburn. Debilitated, sluggish constitution. Desire for fanning due to sensation of oxygen deficiency.

End Antacid Dependency — Heal Naturally
