Heavy Periods (Asrigdara)

Heavy menstrual bleeding — menorrhagia — is bleeding that is heavy enough to affect your daily life, however much that is. It is very common, often goes untreated for years, and is one of the leading causes of anaemia among women in India. Effective treatments exist. Ayurveda calls it Asrigdara or Raktapradara.

Go to hospital urgently if

  • You are soaking a pad every hour for several hours, or passing very large clots
  • You feel faint, dizzy, breathless or have a racing heart
  • You are or could be pregnant and are bleeding heavily or have severe pain
  • Heavy bleeding follows childbirth, a miscarriage or an abortion

Is my bleeding too heavy?

Doctors now define heavy periods by their effect on your life rather than by measuring blood loss. Signs include:

  • Needing to change a pad or tampon every 1–2 hours, or using two at once
  • Changing protection during the night, or bleeding through clothes or bedding
  • Clots larger than about 2.5 cm (a large coin)
  • Periods lasting more than 7 days
  • Missing school, work or social activities because of bleeding
  • Tiredness or breathlessness from anaemia

Herbs such as Shatavari and Ashoka are used in Ayurvedic women’s health. Take them only with qualified guidance, and never during pregnancy without your doctor’s approval.

Heavy periods and anaemia

Every heavy period uses up iron. Over months, iron stores run out and iron-deficiency anaemia develops: tiredness, breathlessness on stairs, headaches, hair loss, pale skin, and a craving to chew ice or eat mud or raw rice (pica). Ask for a haemoglobin and ferritin test. Iron tablets correct the anaemia — take them as prescribed, usually for three months after haemoglobin returns to normal to refill stores — but the bleeding itself also needs treating, or the anaemia returns.

Tests & treatment

A doctor will ask about your periods and general health, examine you, and usually check haemoglobin and thyroid function. A pelvic ultrasound looks for fibroids and polyps; women over 45, or with bleeding between periods, may need a sample of the womb lining (endometrial biopsy). Treatment options include:

  • Hormonal IUD (LNG-IUS) — reduces bleeding by about 90% in many women and also provides contraception; often the first choice
  • Tranexamic acid — taken only on heavy days; reduces blood loss by about half
  • Anti-inflammatory painkillers such as mefenamic acid — reduce bleeding and pain
  • Hormonal tablets — combined pill or progestogens
  • Procedures — removal of polyps or fibroids, endometrial ablation, or hysterectomy when other treatments have not worked and no future pregnancy is wanted

Ayurvedic view

Cool aggravated Pitta and calm Apana Vata → strengthen and tone the uterus → replenish Rakta (blood).

Asrigdara — literally “bursting forth of blood” — is described in Charaka Samhita as excessive or prolonged menstrual bleeding, with four types according to the dosha involved. The Pitta type, with hot, heavy, bright bleeding, is the most common. Salty, sour, spicy, fried and fermented food, alcohol, excessive exertion, repeated pregnancy loss, day sleep and strong emotions are listed as causes. Treatment uses cooling, astringent (kashaya) and blood-stopping (raktasthambhana) herbs, diet correction and rest, followed by blood-nourishing care.

Herbs & safety

  • Ashoka (Saraca asoca) bark — the classical uterine herb for heavy bleeding
  • Lodhra (Symplocos racemosa) — astringent, used for bleeding and discharge
  • Pushyanuga churna — a classical formulation for Pradara
  • Amalaki and Shatavari — cooling and nourishing

Clinical evidence for these herbs is limited. Ashokarishta contains self-generated alcohol. Ashoka bark is frequently adulterated in the market — buy licensed products. Herbs should not delay an ultrasound or, over 45, a biopsy, and should not be used if you might be pregnant.

Self-care

  • Keep a bleeding diary — days, number of pads, clots, flooding — and take it to your appointment.
  • Eat iron-rich foods daily with vitamin C (amla, lemon, guava); avoid tea or coffee with meals.
  • Rest on heavy days; avoid strenuous exercise if you feel light-headed; drink plenty of fluids.
  • During heavy periods, Ayurveda advises cooling foods — rice, milk, pomegranate, coconut water — and avoiding very spicy, sour and fermented food.
  • Avoid aspirin for period pain — it can increase bleeding.
