Erectile dysfunction (ED) is a frequent difficulty getting or keeping an erection firm enough for sex. It is common
— especially after 40 — and treatable. It can also be an early warning of heart disease or diabetes, so it deserves
a proper check-up. Ayurveda addresses ED as Klaibya, through lifestyle, stress care and its
Vajikarana branch of sexual health.
What is erectile dysfunction?
An erection depends on healthy blood vessels, nerves, hormones and a relaxed mind working together. Sexual
arousal signals the blood vessels in the penis to open and fill with blood. A problem in any part of this chain
can cause ED. An occasional difficulty — when tired, stressed or after drinking — is normal. ED is diagnosed when
it happens most of the time over several months.
Slow, unforced breathing and gentle yoga help manage stress. Avoid breath-holding and forceful techniques if you have heart disease, high blood pressure or are pregnant.
Causes
Blood vessel problems — the most common cause in men over 40: diabetes, high blood pressure, high cholesterol, smoking
Hormonal — low testosterone, thyroid disorders, high prolactin
Psychological — performance anxiety, stress, depression, relationship problems; often the main cause in younger men, and it can add to physical causes at any age
A clue: if erections still happen on waking or during sleep, a psychological cause is more likely.
ED and heart health
The penile arteries are narrower than the heart's arteries, so they show damage earlier. ED often appears
3–5 years before a heart attack or angina. Men with new ED should have their blood pressure,
blood sugar and cholesterol checked.
Medical treatment
A doctor will review your history and medicines, examine you and check blood sugar, lipids and, where relevant,
testosterone. Treatment options include:
Treating the underlying causes and switching medicines that may be responsible
Tablets such as sildenafil or tadalafil — effective for most men, but dangerous with nitrate heart medicines, so they must be prescribed
Counselling or sex therapy, individually or as a couple
Vacuum devices, injections, and penile implants for men who don't respond to tablets
Testosterone therapy only when a deficiency is confirmed by blood tests
Ayurvedic view
Nourish Shukra dhatu → balance Vata → strengthen digestion and overall vitality (bala, ojas) → calm the mind.
Charaka Samhita describes four types of Klaibya: caused by depletion of reproductive tissue
(shukra kshaya), by ageing (jara), by disease or injury (dhvajabhanga) and by an
inborn cause (bijopaghata) — along with a clear recognition that fear, grief, anxiety and relationship
strain (manasika causes) can produce it. Vajikarana, one of the eight branches of Ayurveda,
is devoted to sexual health. It prescribes a nourishing diet, a fulfilling relationship, rest and rejuvenating
herbs such as Ashwagandha, Shatavari, Kapikacchu, Gokshura and Safed
Musli, sometimes after a cleansing course. Some of these have small studies showing benefit, particularly for
stress-related ED; they work best alongside treatment of diabetes, blood pressure and other root causes.
Lifestyle steps that help
Stop smoking and chewing tobacco — one of the most effective single steps.
Exercise 30–40 minutes of brisk walking, cycling or swimming most days; add pelvic-floor (Kegel) exercises.
Lose excess weight, particularly around the waist.
Limit alcohol and avoid recreational drugs.
Eat well — vegetables, fruit, whole grains, nuts, seeds, dal; less fried food and sugar. Ayurveda favours milk, ghee, almonds, dates and urad dal in moderation.
Reduce stress with yoga, meditation or pranayama, and talk openly with your partner.
Beware of “power” products
Many “herbal” or “Ayurvedic” stamina capsules sold online, at roadside stalls or through advertisements have
been found to contain hidden sildenafil, tadalafil, steroids or heavy metals. The hidden drugs can drop blood
pressure dangerously — especially with heart medicines. Buy only products with an AYUSH licence number, batch
number and full manufacturer address, ideally prescribed by a registered practitioner, and avoid clinics that
promise guaranteed or “permanent” cures.
When to see a doctor
ED that persists for more than a few weeks, or causes distress
Chest pain or breathlessness on exertion, or known diabetes, high BP or high cholesterol
A curve or lump in the penis, or pain during erections
Reduced sex drive with tiredness, low mood or breast enlargement (possible hormone problem)
Emergency: an erection lasting more than 4 hours — go to hospital immediately
